A basketball tryout evaluation form is a score sheet coaches use to rate every player on the same skills during tryouts. This free PDF lists players by tryout number and rates each one from 1 to 5 on ball handling, shooting, passing, defense, rebounding, athleticism and coachability, with a total and a notes column.

How to use it
- Give every player a numbered jersey or tryout number, and write the numbers and names before tryouts start.
- Use the 1–5 scale on the form (1 = needs work, 3 = team level, 5 = stands out).
- Have each evaluator score on their own sheet, then compare totals together.
- Use the notes column for position, attitude, and anything a score misses.

Skills (ball handling, shooting, passing), defense and rebounding effort, athleticism, and coachability: how a player listens, competes and responds to correction. Rating everyone on the same list keeps it fair.
How do you run a fair basketball tryout?
Use the same drills and the same scoring sheet for every player, have more than one evaluator score independently, and identify players by number so evaluators score what they see.
How do you keep track of players at tryouts?
Give each player a number. Numbered reversible jerseys (for example #0–9 in a 10-pack) make it easy to identify players from the sideline and to split groups for scrimmages.
